Insurance

We accept numerous health plans and cannot know all the details of your policy. It is in your best interest to know and understand your benefits, deductible, co-payments, and authorization requirements before you seek services. If you have questions as to what is covered by your insurance, we recommend that you call your insurance company directly. Co-payments and payments toward your deductible must be made at the time of your visit. We accept cash, check, and most credit cards. Please bring your insurance card to every visit. If you do not have proof of insurance at the time of your visit, you will be expected to pay for the visit at the time. You are responsible for providing us with all current insurance information. Many health plans restrict the hospital, laboratory, or specialist that you may use. The majority also require authorization prior to referral. We may ask that you make an appointment with us prior to referral. It is your responsibility to know the name of your health plan, and to inform us when you have been referred out of our office for services by other providers. Failure to be referred appropriately may result in your insurance company’s denial to pay for the services, which will leave you responsible for payment.
